1. Find your nearest licensed bingo premises by link.
2. Visit in person or call the premises directly.
3. Complete a digital self-exclusion form with staff.
4. Provide a digital photo of yourself.
5. Sign the form to confirm your exclusion period.
6. Your details are added to the national BISES database within 2 working days.
